  • * Grant Parish Disciplinary policy will be followed on all assessment levels.

  • 5. Intervention Plan: Based on the level of risk --
  • Low and/or Medium Level of Threat:  Develop goal and plan to address behavior, teaching, and supporting new behavior.  Set up a timeline to review, plan, and monitor interventions.

  • High Level of Threat: Utilize steps in low/medium level interventions to plan for school re-entry.
  • The parent/guardian notification letter may be filled out and signed for Threat to Harm Others, but must be filled out for Threat To Harm Self. The original should be kept on file. A copy may be emailed to Cathy Horne, School Psychologist, for her to file as well.
