Public ADA Accommodations Request Form

If you have a disability and require a reasonable accommodation to fully participate in a school board meeting please fill out this form and someone will contact you to discuss your accessibility needs. ADA accommodation requests should be received 3-5 days prior to the event for which the accommodations are needed.

The School Board shall allow any member of the public with a disability recognized by the American Disability Act, or a designated caregiver of such person to participate in its meetings via teleconference or video conference, utilizing procedures established by the Superintendent to do so.

Once submitted you will also be given the opportunity to upload documents verifying your disability such as a dr. note, or any other documentation you choose to share.
